A wedding RSVP with a meal and dietary needs for each guest, a shuttle question, a song request and a message for the couple.
What is in the form
- Name(s) as written on your invitation
- Will you be attending? (one of 2)
- Who is coming? (Name, Meal, Allergies or dietary needs for each guest)
- Will you use the shuttle from the hotel? (yes or no)
- A song that will get you on the dance floor
- A message for the couple (long answer)
“Who is coming?”, “Will you use the shuttle from the hotel?” and “A song that will get you on the dance floor” only appear when someone answers “Joyfully accept” to “Will you be attending?”.
It opens with a welcome screen (“Will you join us?”) and ends with a thank-you message you can edit.
After someone submits
This template comes without a workflow. Responses land in your dashboard, and you can add a step in the workflow editor: an approval by email, a post to Slack or Teams, a row in Notion or Airtable, or a webhook.
