Register new members for a sports club: player details, a parent or guardian for juniors, medical notes, consents, and the membership secretary's approval.
What is in the form

- I agree to follow the club’s code of conduct. (yes or no)
- May we use photos of matches and training that include you on the club’s website and social media? (yes or no)
“Other family members” only appears when someone answers “Family” to “Membership type”. “Parent or guardian” and “Their phone” only appear when someone answers “Junior (under 18)” to “Membership type”.
It opens with a welcome screen (“Join the club”) and ends with a thank-you message you can edit.
After someone submits
An approval by email (Pro). Each submission goes to an approver you name, with the answers and two buttons: “Approve” and “Decline”. The rest of the workflow follows their decision.
The workflow comes with the form, switched off until you fill in the blanks: the approval needs Pro.
